Formstack
A workflow suite first, a form builder second.
Formstack, now operating under the Intellistack name, is less a form tool than a workflow platform. Its forms feed document generation, e signatures, and governed back office processes, with HIPAA options for regulated teams. That makes it powerful where a form is one step in a larger pipeline, and expensive where you only need to collect a few responses. There is no permanent free plan, only a trial, and the builder feels more enterprise than friendly.

Powerful in a pipeline, costly on its own
Formstack scores high on features and integrations and lower on value and ease of use. The verdict tracks the use case: excellent when forms drive documents and approvals, hard to justify when you only need to collect responses.
Scored against the same five weighted criteria we use across form builders. See the rubric →

you want a low cost or free way to collect responses without the surrounding workflow suite.
