A workflow suite first, a form builder second.

Formstack, now operating under the Intellistack name, is less a form tool than a workflow platform. Its forms feed document generation, e signatures, and governed back office processes, with HIPAA options for regulated teams. That makes it powerful where a form is one step in a larger pipeline, and expensive where you only need to collect a few responses. There is no permanent free plan, only a trial, and the builder feels more enterprise than friendly.
